Short answer
Without insurance, the cheapest weight loss injection is compounded semaglutide at $69/month (12-month prepay) or $119/month billed monthly. The cheapest FDA-approved brand is Wegovy at $349/month direct from NovoCare or Costco. Everything in between is a trade-off between price, commitment and the provider's complaint record — and that last one is where cheap shots get expensive. Prices verified October 2026.

The $69 question: is prepay worth it?
$69 vs $119 is $600 a year. That is real money. But the $69 rate requires $828 upfront, is non-refundable if you stop for side effects or life, and Amble's BBB file shows what happens when a prepaid provider under-ships. Our rule: prepay only after one month at a monthly provider proved you tolerate the medication, and only with a company whose complaint file you have read. The refund exposure by provider is listed here.
Cheapest FDA-approved shot: brand-name direct
If you want the product that was in the clinical trials, $349 is the number. NovoCare sells Wegovy and Ozempic at $349/month to anyone with a prescription ($199 for the first two months for new patients). Costco matches it for members, with Sesame providing the prescription for ~$59/month. LillyDirect sells Zepbound in single-dose vials at $349 (2.5 mg), $399 (5 mg) and $499 (7.5-15 mg). Compare them in NovoCare vs LillyDirect.
What "cheapest" should not mean
- No prescription required. Semaglutide and tirzepatide are prescription-only. A site that skips the prescriber is selling an unregulated or counterfeit product.
- "Research peptides." Vials labeled "not for human use" are not pharmacy-compounded and have no sterility or potency testing.
- Prices under ~$69/month. Below the ingredient-plus-pharmacy cost floor, something is missing — usually the medication.
- Unnamed pharmacies. A legitimate provider names its 503A/503B pharmacy and can show a Certificate of Analysis.
Free and near-free routes
- Manufacturer patient assistance — $0 for qualifying uninsured low-income patients; Wegovy/Zepbound access is restricted.
- Medicare Bridge — $50/month cap if you are a low-income Part D beneficiary.
- Clinical trials — free medication for participants; see retatrutide and orforglipron trial pipelines.
