Qualifying weight-related conditions

If your BMI is between 27 and 30, you need at least one of the following conditions to qualify:

  • High blood pressure (hypertension) — systolic ≥ 130 or diastolic ≥ 80, or on blood pressure medication
  • Type 2 diabetes — HbA1c ≥ 6.5%, or on diabetes medication
  • High cholesterol (dyslipidemia) — elevated LDL, low HDL, or on lipid-lowering medication
  • Obstructive sleep apnea — diagnosed or symptomatic (loud snoring, daytime sleepiness)
  • Cardiovascular disease risk factors — family history, smoking, or other risk factors

Who should NOT take GLP-1 medications (contraindications)

The following are absolute or relative contraindications for GLP-1 medications:

  • Medullary thyroid carcinoma (MTC) — personal or family history. GLP-1 medications carry a boxed warning for thyroid C-cell tumors.
  • MEN 2 syndrome — Multiple Endocrine Neoplasia type 2.
  • Pregnancy — GLP-1 medications should be stopped at least 2 months before planned pregnancy.
  • Breastfeeding — it is not known if GLP-1 medications pass into breast milk.
  • Pancreatitis — history of acute or chronic pancreatitis.
  • Severe gastrointestinal disease — including gastroparesis, as GLP-1s slow gastric emptying further.
  • Known hypersensitivity — allergy to semaglutide, tirzepatide, or formulation ingredients.

Conditions that require caution (not absolute contraindications)

  • Diabetic retinopathy — GLP-1s may worsen retinopathy in some patients; consult your eye doctor.
