The answer in two sentences
Mounjaro (tirzepatide) lowers A1c by 1.9 to 2.6 percentage points depending on dose and trial — the largest reduction of any non-insulin diabetes medication, and larger than basal insulin in head-to-head trials. It beat semaglutide 1 mg (Ozempic) directly in SURPASS-2 on both A1c and weight. Data below is from the five pivotal SURPASS trials; cost figures verified October 2026.
Mounjaro A1c reduction by dose and trial
| Trial (duration) | Comparator | Mounjaro 5 mg | 10 mg | 15 mg | Comparator result |
|---|---|---|---|---|---|
| SURPASS-1 (40 wk) | Placebo, monotherapy | -1.87 | -1.89 | -2.07 | +0.04 |
| SURPASS-2 (40 wk) | Semaglutide 1 mg | -2.01 | -2.24 | -2.30 | -1.86 |
| SURPASS-3 (52 wk) | Insulin degludec | -1.93 | -2.20 | -2.37 | -1.34 |
| SURPASS-4 (52 wk) | Insulin glargine | -2.24 | -2.43 | -2.58 | -1.44 |
| SURPASS-5 (40 wk) | Placebo, add-on to glargine | -2.11 | -2.40 | -2.34 | -0.86 |
Baseline A1c in these trials was 7.9-8.5%, so most participants finished in the non-diabetic or prediabetic range. In SURPASS-2, 81-86% reached A1c under 7% on tirzepatide versus 79% on semaglutide, and 27-46% reached under 5.7% (the normal range) versus 19%.

How fast it works
- Weeks 1-2 (2.5 mg): fasting glucose begins falling; appetite drops noticeably.
- Week 12: first meaningful A1c reading — most of the eventual reduction is already visible.
- Weeks 12-40: continued improvement as the dose climbs to 10-15 mg; weight loss contributes to insulin sensitivity.
Dose schedule: 2.5 mg for 4 weeks (non-therapeutic starter), then 5 mg, with optional 2.5 mg steps every 4+ weeks to 15 mg. See the Mounjaro dosage guide.
Side effects and daily life
The trade-off for the A1c result is gastrointestinal: nausea (12-18%), diarrhea (12-17%), decreased appetite, vomiting and constipation, concentrated during titration and mostly resolving by week 8-12. Discontinuation for side effects ran 5-9% across SURPASS. Hypoglycemia is rare unless combined with insulin or a sulfonylurea, whose doses are usually reduced. Most people describe energy improving after the first month as glucose stabilises and weight drops; the first weeks can include fatigue. Full list in Mounjaro side effects; fatigue specifically in GLP-1 fatigue.

Is Mounjaro right for your diabetes?
- Strong fit: type 2 diabetes with A1c above goal on metformin, especially with BMI over 27, or when weight gain from insulin or sulfonylureas is a problem.
- Cardiovascular disease: SURPASS-4 showed no increase in cardiovascular events; the dedicated outcomes trial (SURPASS-CVOT) compared it to dulaglutide. Semaglutide has the longer established cardiovascular-benefit label.
- Kidney disease: no dose adjustment; GLP-1s slow albuminuria progression.
- Not suitable: type 1 diabetes, history of pancreatitis, personal or family history of medullary thyroid cancer or MEN2, pregnancy.
A prescriber decides based on your A1c trajectory, other medications and kidney/liver function; read Mounjaro for diabetes for the clinical overview.
