Chest Pain on Ozempic or Wegovy: When to Worry on GLP-1 (2026)
Most GLP-1 chest pain is reflux from slowed gastric emptying — but the red flags that need the ER are different. The triage framework.

Short answer
Usually GI, always worth triaging. GLP-1 medications don't directly cause cardiac chest pain — semaglutide actually reduces cardiovascular events in trials. But slowed gastric emptying creates reflux that produces very convincing chest discomfort, and GLP-1 users often carry cardiac risk factors. Treat every episode as a triage problem: rule out the dangerous causes first.
The likely culprit: reflux masquerading as chest pain
Delayed gastric emptying + reduced food volume = food and acid sitting in the stomach longer, with more opportunity to reflux into the esophagus. On GLP-1s this produces:
- Burning/pressure behind the sternum — often after eating or lying down
- Sour taste or food 'coming back up' — classic reflux signature
- Relief with antacids or standing — a useful distinguishing feature
- Worse during early titration — as gastric slowing is strongest
This is usually benign and manageable — but it feels alarming because it mimics cardiac pain. Our GERD guide and heartburn guide cover management.
The cardiac red flags — go to ER
- Crushing/pressure pain radiating to arm, jaw, back
- Chest pain + shortness of breath, sweating, nausea, dizziness
- Palpitations + chest pain + faintness (arrhythmia, not reflux)
- Pain lasting >5 minutes unrelieved
- Any chest pain with existing heart disease or diabetes complications
Don't self-diagnose "it's probably the GLP-1." The user base for these drugs skews toward exactly the population where real chest pain happens — obesity, hypertension, diabetes, metabolic syndrome. Reflux is the common answer; cardiac is the dangerous one. Rule out the dangerous first.
The GLP-1 heart-rate effect
A small resting heart rate increase (~1-4 bpm) is documented — probably reflex response to the metabolic changes. Alone, it's benign. Combined with chest pain, dizziness, or faintness, it could signal something needing workup. The distinction: palpitations alone, monitor; palpitations + other symptoms, evaluate.
Managing the reflux-type pain
- Smaller, slower meals — the slowed stomach tolerates less volume
- No food 2-3h before bed — lying down with a full slow stomach triggers reflux
- OTC antacids/H2 blockers — usually effective for episodic relief
- Discuss dose timing — some providers adjust injection timing for GI-side-heavy users
