GLP-1 Sick Day Rules: What to Do When You're Ill on Ozempic (2026)

Short answer

Hold the dose if you can't keep fluids down. Missing one weekly GLP-1 dose during illness doesn't erase progress — but taking it while actively vomiting or dehydrated can land you in the ER. The dangerous part of being sick on GLP-1 isn't the virus; it's the dehydration you won't feel coming.

The hold-or-take decision

SituationAction
Mild cold, eating/drinking OKTake dose normally
Low appetite, minor GI upset, hydratedUsually OK to take; monitor hydration
Active vomiting or diarrheaHold — restart when eating/drinking normalizes
Can't keep fluids downHold — this is the dehydration trigger situation
Severe illness, any hospitalizationHold — providers will sort restart timing

Why dehydration is the real enemy

GLP-1s suppress thirst alongside appetite — you can be badly dehydrated without feeling thirsty. Add illness fluid losses (fever, vomiting, diarrhea) and the gap widens fast. Dehydration on GLP-1 is the most common reason users end up hospitalized during a routine illness. It also makes NSAIDs kidney-toxic, makes orthostatic dizziness worse, and deepens the fatigue the illness already causes.

The rule: small sips constantly, not big gulps when you feel bad. An electrolyte solution sipped all afternoon beats a glass of water forced down twice. Our electrolytes guide covers formulations.

What to eat when you can't eat

  • First priority — fluids: electrolyte solution, broth, diluted juice, ginger ale flat, popsicles
  • Then easy protein: bone broth, protein shake sips, scrambled eggs, Greek yogurt
  • Then bland carbs: toast, rice, crackers, applesauce
  • Skip: anything fatty, fried, spicy, or large — your slowed stomach will return it

Don't force normal meals — a few days of minimal intake is survivable; dehydration is not. When appetite returns, ramp gradually rather than catching up.

When sick-day symptoms are actually the drug

Overlap trap: nausea, vomiting, abdominal pain, and fatigue are GLP-1 side effects AND illness symptoms. The distinguishing clues: side effects track dose timing (peak days 1-3 after injection, improve through the week), tend to be nausea-dominant without fever, and don't produce the systemic sick feeling. If symptoms persist past the usual 2-3 day post-injection window, worsen, or include fever — treat it as illness, not side effects.

The call-the-doctor thresholds

  • Can't keep any fluids down for 12+ hours
  • Severe dehydration signs: dark/scant urine, dizzy standing, racing heart, confusion
  • Fever >103°F or lasting >3 days
  • Severe abdominal pain — especially right upper quadrant (gallbladder) or radiating to back (pancreatitis)
  • Blood in vomit or stool
  • Worsening past day 3 instead of plateauing
