Why GLP-1 medications cause diarrhea

Diarrhea is the third most common GLP-1 side effect, affecting approximately 30% of Wegovy users and 23% of Zepbound users in clinical trials. It occurs because GLP-1 receptor agonists slow gastric emptying and alter gut motility, which can disrupt normal digestion and lead to loose, frequent stools.

9 ways to manage GLP-1 diarrhea

1. Follow the BRAT diet

Bananas, rice, applesauce, and toast are easy to digest and help firm stools. Eat these for 1-2 days during acute diarrhea episodes.

2. Avoid sugar alcohols

Xylitol, sorbitol, and maltitol (found in sugar-free gum, protein bars, and keto products) are common diarrhea triggers. Check labels carefully.

3. Stay hydrated

Drink 8-10 glasses of water daily. Add electrolyte drinks (low-sugar) if diarrhea is frequent. Dehydration is the main risk.

4. Take loperamide (Imodium)

OTC loperamide is safe with GLP-1s. Take 2mg after each loose stool, max 8mg/day. Do not exceed 2 days without consulting your provider.

5. Eat soluble fiber

Soluble fiber (psyllium husk, oats, chia seeds) absorbs water and firms stools. Start with 1 teaspoon daily and increase gradually.

6. Avoid greasy and spicy foods

High-fat foods worsen diarrhea by speeding up gut transit. Stick to lean proteins and simple carbohydrates during flare-ups.

7. Try probiotics

Lactobacillus and Bifidobacterium probiotics may help restore gut balance. Look for supplements with 10+ billion CFUs.

8. Limit caffeine and alcohol

Both stimulate gut motility and worsen diarrhea. Switch to herbal tea and water during episodes.

9. Talk to your provider about dose adjustment

If diarrhea is persistent, your provider may slow your titration schedule or temporarily reduce your dose.
