Why GLP-1 medications cause vomiting

Vomiting occurs because GLP-1 receptor agonists slow gastric emptying. Food stays in the stomach longer, which can trigger the vomit reflex. The effect is strongest during the first 4 weeks and after each dose increase. Semaglutide (Wegovy) causes more vomiting than tirzepatide (Zepbound) due to differences in receptor activity.

7 ways to prevent and manage GLP-1 vomiting

1. Eat small, frequent meals

5-6 small meals per day instead of 3 large ones. Smaller meals empty faster, reducing vomit risk.

2. Inject at bedtime

Many patients report less vomiting when injecting at bedtime, sleeping through the peak side effect window.

3. Follow the BRAT diet during flare-ups

Bananas, rice, applesauce, and toast are gentle on the stomach. See our foods to avoid guide.

4. Stay hydrated with small sips

Sip water, electrolyte drinks, or ice chips. Do not chug large amounts, which can trigger vomiting.

5. Avoid trigger foods

Greasy, spicy, very sweet, and strong-smelling foods are common triggers. See our nausea remedies guide for the full list.

6. Ask about ondansetron (Zofran)

If vomiting is severe, your provider can prescribe ondansetron. It is safe with GLP-1 medications and highly effective.

7. Do not rush titration

Follow your provider's dose schedule. If vomiting is severe at a new dose, ask about staying at the current dose longer before increasing.
