The biggest GLP-1 access change in Medicare history
On July 1, 2026, CMS launched the Medicare GLP-1 Bridge: a demonstration program giving eligible Part D beneficiaries access to weight-loss GLP-1s for a flat $50/month. An estimated 14 million people could qualify. It runs through December 31, 2027.

The BMI tiers: who qualifies
| Tier | Requirement |

| Tier 1 | BMI 35+ at start of GLP-1 therapy |
| Tier 2 | BMI 30+ with heart failure, uncontrolled hypertension (2+ meds), or CKD stage 3a+ |
| Tier 3 | BMI 27+ with prediabetes, prior heart attack, prior stroke, or symptomatic PAD |
You must also be 18+, enrolled in Medicare Part D (standalone drug plan or Medicare Advantage with drug coverage), and the prescription must be for weight reduction and maintenance with lifestyle modification.
The fine print most articles bury
- The $50 doesn't count toward your deductible or the $2,100 out-of-pocket cap. It's a parallel program outside the Part D payment flow.
- Extra Help can't reduce it. Low-income subsidies don't apply; $50 is $50 for everyone.
- No stacking. Manufacturer savings cards cannot combine with the Bridge.
- KwikPen or nothing for Zepbound. A vial or single-dose pen prescription will be rejected at the pharmacy.
- It ends December 31, 2027. A successor model (BALANCE) is planned but not guaranteed. Starting now could mean losing the price in 2028.
How to actually get it
- Confirm you have Part D coverage (call 1-800-MEDICARE if unsure).
- Ask your doctor for a Bridge-eligible prescription — specify Zepbound KwikPen if that's your drug.
- Your prescriber submits prior authorization to the CMS central processor (not your Part D plan).
- Pick up at the pharmacy, pay $50. Refills need no new approval while you stay on the same drug.
Not on Medicare? Your cheapest paths
The Bridge is Medicare-only. If you're under 65 or on commercial insurance, the cheapest legitimate routes are manufacturer self-pay (NovoCare, LillyDirect at $149-$449/mo), warehouse programs (Costco, Sam's Club), or telehealth compounded options.
