The "Zepbound savings card" advertised as low as $25 is real, but that price applies to one specific situation. Lilly runs parallel tiers based on your insurance status, and landing in the wrong assumption can mean a $499 surprise instead of $25. Here is every tier, who qualifies, and the fine print that matters.

The fine print that trips people up
- "Covered" vs "not covered" is the $474 question. Having insurance isn't enough; your plan must list Zepbound on formulary. Check before assuming $25.
- The 45-day refill rule. On self-pay KwikPen/vial tiers, doses 7.5 mg and up hold $449 only if you refill within 45 days. Miss it and you're at $499 to $699.
- Government plans are excluded. Medicare, Medicaid, TRICARE, VA, IHS: none qualify for the commercial card. The Medicare Bridge ($50/mo, KwikPen only) is the exception for eligible Part D beneficiaries.
- Fill caps. 13 fills/year on pen tiers, 11 on KwikPen. Roughly one year of coverage either way.
- Card expires 12/31/2026. Renewal terms can change.
