Mounjaro in Vials: Does It Exist in the US, and What You Can Actually Buy

Mounjaro in Vials: Does It Exist in the US, and What You Can Actually Buy

Short answer

Mounjaro is not sold in vials in the US. It comes only in single-dose KwikPen auto-injectors. The tirzepatide vial is Zepbound — the same molecule, labelled for weight loss and sleep apnea — sold as single-dose glass vials through LillyDirect at $349-$499/month depending on dose. The third option is compounded tirzepatide in multi-dose vials at $119-$299/month from licensed telehealth providers. Anything labelled "Mounjaro vial" is either compounded product mislabeled or counterfeit.

Why Zepbound gets vials and Mounjaro does not

Eli Lilly launched Zepbound single-dose vials in 2024 as a self-pay product to pull self-pay patients away from compounding pharmacies. Manufacturing a glass vial is cheaper than a pen, which is how Lilly can sell it at $349-$499 against a $1,086 pen list price. Mounjaro is aimed at insured patients whose plans pay list, so a discounted vial version would cannibalise its own margin. Same molecule, two channels, two prices.

How to use a tirzepatide vial safely

Zepbound single-dose vial

  1. Check the vial: clear, colourless solution; no particles; not expired.
  2. Swab the rubber stopper with alcohol.
  3. Draw the entire contents (0.5 mL) into the insulin syringe provided.
  4. Inject subcutaneously in abdomen (2 inches from navel), front of thigh, or back of upper arm; rotate weekly.
  5. Discard the vial — single dose means no leftovers, ever.

Compounded multi-dose vial

  1. Note the concentration on the label (10, 15, 17, 20, 30 mg/mL are common).
  2. Convert your mg dose to units: dose ÷ concentration × 100 (full chart in tirzepatide dosing in units).
  3. Draw exactly that volume with a U-100 insulin syringe.
  4. Refrigerate between uses; standard beyond-use dating is 28 days after first puncture unless your pharmacy's label says otherwise. See the GLP-1 storage guide.

Vial vs pen vs compounded: which to choose

  • Pen (Mounjaro/Zepbound retail): easiest to use, pre-set dose — but list-price-only without insurance.
  • Zepbound vial (LillyDirect): brand-name product at roughly one-third of list; you draw it yourself. Best for people who want FDA-approved tirzepatide without insurance.
  • Compounded vial: cheapest at $119-$299/month and dose-flexible for micro-adjustments, but not an FDA-approved finished product and requires measuring your own dose. Pharmacy quality is everything — see compounded tirzepatide vs Mounjaro.

Counterfeit warning

The FDA has warned about counterfeit and unapproved tirzepatide sold online. Signals that a "Mounjaro vial" is not legitimate: no prescription required, shipped from overseas, price under ~$100 for brand-labelled product, or packaging that does not match Lilly's official vial. Verify with how to spot fake GLP-1 products, and read why salt forms are a red flag.
