Orforglipron dosing at a glance

QuestionAnswer
Route / frequencyOral tablet, once daily
Starting dose1 mg daily
Titration1 → 3 → 6 → 12 → 36 mg, ~4 weeks per step as tolerated
Maintenance doses studied6 mg, 12 mg, 36 mg
Maximum dose36 mg daily
Food / water rulesNone — with or without food
Time of dayAny, consistently
Missed doseTake same day if remembered; otherwise skip, never double
With insulin / sulfonylureaAllowed; reduce insulin/SU dose to limit hypoglycemia
Weight loss (36 mg, 72 wk)~12.4% (ATTAIN-1)
A1c reduction (T2D, 40 wk)~1.3-1.6 points (ACHIEVE-1)

Dose steps and outcomes reflect the published phase 3 ATTAIN and ACHIEVE protocols. Where the final FDA label differs, the label governs. This page is educational, not a substitute for the prescribing information.

How to titrate orforglipron

The escalation schedule is slow by design. GLP-1 nausea is dose-dependent and front-loaded, so each step is held roughly four weeks before increasing. A typical course:

WeeksDosePurpose
1-41 mgTolerability only; little weight effect
5-83 mgEarly appetite effect
9-126 mgFirst maintenance-eligible dose
13-1612 mgMid maintenance dose
17+36 mgMaximum dose; best efficacy

Patients who hit their goal or plateau with acceptable side effects can hold at 6 mg or 12 mg rather than escalating. If a step causes significant nausea or vomiting, dropping back one level for 2-4 weeks and re-attempting is standard GLP-1 practice. See the nausea management guide.

When to take it: timing, food and water

Orforglipron is a non-peptide small molecule, so it is absorbed through the gut like a conventional tablet. That is why it has none of the Rybelsus rules: no empty stomach, no 4-oz water limit, no 30-minute wait. Take it at a consistent time; morning works for most people because any nausea peaks during the day rather than overnight. If nausea is a problem, some patients do better taking it with a small meal.

Missed dose

Same-day: take it when remembered. Next day: skip, resume the normal schedule, never take two to catch up. A single missed day has little clinical effect because the drug has been accumulating at steady state. Missing a week or more during titration generally means re-starting at the previous tolerated step rather than jumping back to the current one.

Combining with insulin, sulfonylureas and other diabetes drugs

  • Basal insulin: reduce by about 10-20% at initiation and re-titrate to fasting glucose; GLP-1s lower glucose and appetite simultaneously, so insulin needs fall.
  • Sulfonylureas: halve the dose or stop; the hypoglycemia risk from the combination is the main reason.
  • Metformin, SGLT2 inhibitors, DPP-4 inhibitors: no dose change needed; DPP-4 inhibitors are usually stopped because they are redundant with a GLP-1. See GLP-1 + SGLT2.

Drug interactions that matter

Published trial data has not flagged major cytochrome-mediated interactions. The clinically relevant interaction is the class effect: slowed gastric emptying can change absorption timing of oral drugs with a narrow therapeutic window — levothyroxine, warfarin, some antiepileptics and oral contraceptives. The usual advice is to monitor (INR, TSH, seizure control) during titration and consider separating doses. Full list in GLP-1 drug interactions.

Gallbladder and other class risks

Gallstones and cholecystitis occur more often with GLP-1 therapy, largely because rapid weight loss itself causes them. Trial rates for orforglipron were low and comparable to other agents. Counsel patients with prior gallstones; a history of pancreatitis or medullary thyroid carcinoma / MEN2 remains a contraindication across the class. Read GLP-1 and gallstones.

When to switch from semaglutide (or Rybelsus) to orforglipron

  • Needle refusal or injection fatigue — the primary reason. Expected weight loss is a step down from Wegovy (~15%) and well below Zepbound (~21%), so set expectations.
  • Rybelsus adherence failure — patients who cannot manage the fasting routine. Orforglipron removes it entirely.
  • Cost — Foundayo at $149/month undercuts every brand-name injectable; compounded semaglutide at $119-$199 remains the cheapest route overall.
  • Not a reason to switch: plateau on semaglutide. If weight loss has stalled on Wegovy 2.4 mg, tirzepatide is the more potent option, not orforglipron.

Switching mechanics: stop the weekly injection, wait for the next scheduled injection date (one week), and start orforglipron 1 mg. Because titration restarts from the bottom, patients coming off a high semaglutide dose should expect 8-12 weeks of weaker appetite control.
