The one formula you need
Compounded semaglutide is prescribed in milligrams but drawn in syringe units. The bridge between them is your vial's concentration (mg/mL), printed on the label:
units = (dose in mg × 100) ÷ concentration (mg/mL)
mg = (units × concentration) ÷ 100
On a standard U-100 insulin syringe, 100 units = 1 mL, so 1 unit = 0.01 mL. A "unit" measures volume, not drug. That is why the same 20 units can be 0.5 mg or 2 mg depending on the vial.

If your math lands above 100 units, the dose exceeds a standard 1 mL syringe. Higher doses are normally drawn from more concentrated vials; recheck the concentration with your pharmacy.

The mistake that sends people to the ER
The FDA has documented hospitalizations from exactly this error: a patient draws "50 units" assuming a 2.5 mg/mL vial (1.25 mg) but the vial is actually 10 mg/mL, delivering 5 mg, a 4x overdose. Symptoms: severe nausea, vomiting, dehydration.
Before every injection:
- Read the mg/mL on the vial label. Do not assume it matches last month's.
- Run the formula or check the chart for YOUR concentration.
- If the pharmacy changed concentration between refills, your unit count changes too.
Switching between compounded and brand
Brand pens (Ozempic, Wegovy) dial in mg directly, no units involved. To switch, convert your usual units to mg first, then match to the brand ladder: Wegovy runs 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg. Your calculated dose will often fall between steps; your prescriber decides which way to round based on tolerance.
