Can You Use Expired Ozempic? What the Expiry Date Really Means (2026)

Short answer

Don't — but not because it's toxic. Expired semaglutide doesn't become dangerous; it degrades, meaning you're injecting an unknown fraction of the dose. The bigger rule most people actually violate isn't expiry at all — it's the 56-day limit on opened pens and the shorter beyond-use dates on compounded vials.

What expiry actually guarantees

The printed expiry date is the manufacturer's guarantee that the pen delivers full potency and sterility until that date, stored correctly. Past it:

  • Potency drifts down — gradually, not on a cliff. Weeks past expiry kept cold likely still works near-spec; months past, meaningfully degraded
  • Sterility becomes unverifiable — the preservative system ages too; multi-use pens carry more risk than single-use
  • You can't tell at home — no test exists; the only visible signals are cloudiness, particles, or discoloration

The rule people actually break: 56 days

Once an Ozempic pen is in use, it's discarded after 56 days — fridge or room temp, expiry date irrelevant. A pen can be perfectly inside its expiry window and still need to go at day 57. Wegovy pens are single-use; Zepbound vials and Mounjaro pens have their own labeled windows. Write the discard date on the pen the day you start it.

Compounded is a different clock

Compounded semaglutide and tirzepatide vials carry a beyond-use date (BUD)set by the dispensing pharmacy under USP rules — typically 28-90 days, far shorter than brand pens. A multi-dose vial with bacteriostatic water gets the longer end; preservative-free formulations get less. These aren't conservative estimates — they're the pharmacy's actual stability commitment. Use within the BUD or reorder.

Storage mistakes that kill potency early

  • Freezing — once semaglutide freezes, it's done even after thawing. Back-of-fridge cold spots are the usual culprit
  • Heat — a pen left in a hot car loses potency faster than its date suggests
  • Room-temp beyond label — Ozempic tolerates ≤86°F for the 56-day window only, not indefinitely

When an expired pen is the only option

If you're mid-titration with a pen a few weeks past date, properly refrigerated, clear solution — the realistic harm is a weak dose, not poisoning. It's a judgment call with imperfect information. The clean answer is still no: reorder. If supply or cost is the barrier, that's a real problem worth solving — compare compounded options rather than stretching a degraded pen.
