Can You Use HSA/FSA for GLP-1? Yes — Here's How (2026 Rules)

The discount hiding in your benefits

Everyone hunts for GLP-1 coupons. Almost nobody uses the one that works on every purchase: pre-tax HSA/FSA dollars. At a 22% tax bracket, every GLP-1 dollar costs you 78 cents. At 32%, it costs 68 cents. On a $349/month medication, that is $924 to $1,340 back per year — no eligibility lottery, no savings card terms.

The IRS rule that makes it work

IRS Publication 502: you can deduct amounts paid to lose weight "if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease)." Prescription medications are categorically qualified. A GLP-1 prescribed for a documented diagnosis clears the bar.

What's eligible

ExpenseEligible?Documentation to keep
Brand GLP-1 (Wegovy, Zepbound, Ozempic, Mounjaro)YesRx + itemized receipt
Compounded semaglutide/tirzepatideYes, with docsRx + receipt + LMN recommended
Telehealth visit/consult feesUsuallyItemized receipt + diagnosis
Program membership feesGray zoneLMN strongly recommended
OTC supplements (berberine, etc.)No

How to pay: two methods

  1. Direct card swipe. If the pharmacy or provider accepts HSA/FSA cards, use it. Done.
  2. Pay and reimburse. More common for telehealth. Pay with a personal card, keep the itemized receipt, submit a claim through your administrator's portal (HealthEquity, Fidelity, Optum). Most process in 3-10 business days.

The documentation checklist

  • Itemized receipt: medication name, dose, cost, date
  • Prescription record naming the diagnosis (obesity E66.01, T2D, hypertension)
  • Letter of Medical Necessity — request it when the prescription is written
  • Visit notes for telehealth consults
  • Lab results if ordered

HSAs are self-certified — nobody pre-approves your purchase. But if the IRS audits, this file is your defense. FSAs are reviewed more actively; expect documentation requests on weight-management claims.
