UHC + OptumRx: how coverage actually works
UnitedHealthcare is the largest health insurer in the US, and its pharmacy benefits run through OptumRx. Whether your GLP-1 is covered depends on two layers: the OptumRx formulary (which drugs are on the list) and your employer's plan design (whether weight-loss drugs are included at all). UHC covers GLP-1s for type 2 diabetes on essentially all plans — weight loss is where it gets complicated.

The OptumRx prior authorization checklist
Most UHC/OptumRx GLP-1 approvals require all of the following:
- BMI 30+, or BMI 27+ with hypertension, T2D, dyslipidemia, or obstructive sleep apnea
- Documented lifestyle program: 3-6 months of diet/exercise participation — this is the #1 reason for denial, so have your doctor document it explicitly
- Step therapy on some plans: trial of phentermine, Contrave, or orlistat first
- Prescriber attestation that the drug is for an FDA-approved indication
The sleep apnea workaround
