Why "does Blue Cross cover it" has no single answer

Blue Cross Blue Shield is not one insurer — it's 36 independent companies (Anthem, HCSC, CareFirst, Florida Blue, Premera, etc.), and each sets its own formulary. On top of that, most people with BCBS are on employer-sponsored plans, where the employer decides whether weight-loss drugs are covered at all. Two people with "Blue Cross" cards can get opposite answers.

How to check YOUR plan in 10 minutes

  1. Find your plan's formulary (Google "[your BCBS plan name] formulary 2026" or log into your member portal).
  2. Search for "Wegovy" and "Zepbound" — check the tier and whether "PA" (prior authorization) is listed.
  3. Look for an "anti-obesity exclusion" in your plan documents — if it's there, no GLP-1 for weight loss will be covered regardless of medical need.
  4. Call the member number and ask: "Is Wegovy covered for weight loss on my specific plan, and what are the prior authorization criteria?" Get the reference number.

If you're covered: what you'll pay

  • Typical copay: $25-$100/month (preferred brand tier) or 20-40% coinsurance on non-preferred tiers.

If BCBS denies you: the appeal that works

Denials split into two types. Formulary exclusion ("we don't cover weight-loss drugs") is a plan design choice — appeals almost never succeed because there's no medical-necessity argument to make. Medical necessity denial ("you don't meet criteria") is winnable: submit BMI history, comorbidity documentation (labs, diagnoses), records of prior weight-loss attempts, and a physician letter citing FDA indication.
