Mounjaro Savings Card 2026: $25 Copay Rules, Eligibility & Limits

The $25 promise — and who it actually applies to

Lilly advertises Mounjaro "as low as $25/month" — but that price requires commercial insurance that already covers Mounjaro. The savings card is a copay card: it pays down your existing copay, it doesn't replace insurance. Uninsured or government-covered? Different rules entirely.

Eligibility: the three buckets

Your insuranceCard works?Your Mounjaro price
Commercial plan that covers MounjaroYes$25/mo (up to monthly max benefit)
Commercial plan that excludes itNoLillyDirect Zepbound $349-$499/mo
Medicare / Medicaid / TRICARE / VA / FEHBNo (federal ban)Plan copay (T2D) or Bridge $50 (Zepbound)
UninsuredNoLillyDirect self-pay $349-$499/mo

Mounjaro vs Zepbound: same drug, different card

Mounjaro (diabetes brand) and Zepbound (weight-loss brand) are both tirzepatide — but they run separate savings programs. If you're prescribed Mounjaro for T2D, use the Mounjaro card. If you're prescribed Zepbound for weight loss or sleep apnea, use the Zepbound savings card — which has different tiers including self-pay pricing. Don't mix them up at the pharmacy.

How to use it (5 minutes)

  1. Go to Mounjaro.com and activate the savings offer — you'll get a card with BIN/PCN/Group numbers.
  2. Bring it to the pharmacy with your insurance card.
  3. Insurance runs first, then the card drops the copay to ~$25 (up to the monthly max).
  4. Refills work automatically while the offer is active — reactivate if it expires.

The fine print

  • Monthly max benefit: the card caps its contribution — high-deductible plans can exceed it early in the year.
  • Annual cap: total yearly benefit is limited; check current terms.
  • Government ban is absolute: Medicare, Medicaid, TRICARE, VA, and FEHB beneficiaries cannot use it. See our federal employee guide and TRICARE guide.
  • T2D indication: Mounjaro is diabetes-indicated — using it for weight loss alone is off-label and often denied.
