Federal Employee GLP-1 Coverage 2026: FEHB, FEPBlue & GEHA Rules

FEHB: the biggest employer plan in America

The Federal Employees Health Benefits program covers ~8 million people across dozens of competing plans — FEPBlue, GEHA, MHBP, NALC, APWU, and regional HMOs. Unlike a single-employer plan, you can switch carriers every Open Season, which makes FEHB uniquely navigable for GLP-1 coverage: if your plan excludes weight-loss drugs, you can leave it.

All FEHB plans cover GLP-1s for type 2 diabetes. Weight-loss coverage is where they diverge — always verify the current formulary before assuming.

The savings card ban federal workers miss

Manufacturer copay cards (Wegovy $25 card, Zepbound $25 card) are prohibited for federal healthcare beneficiaries — and OPM treats FEHB as included. Pharmacies will reject the card when they see FEPBlue/GEHA coverage. Your plan copay is your final price. The exception: if your plan excludes the drug entirely and you pay 100% cash, you may use manufacturer self-pay pricing (NovoCare $149-$349, LillyDirect $349-$499) — that's a cash program, not a copay card.

The Open Season strategy

  1. October-November: pull next year's formularies for FEPBlue, GEHA, and MHBP. Search "Wegovy" and "Zepbound."
  2. Compare PA criteria — some plans require 6 months of documented lifestyle program, others 3.
  3. Factor the deductible: an HDHP with HSA can still win if you're funding the HSA anyway — GLP-1s are HSA-eligible expenses.
  4. Switch in Open Season (mid-Nov to mid-Dec) — coverage starts January 1.
